Cremation form B is an application for cremation which must be completed and signed by the next of kin of the deceased or the executor of the will.
Cremation Form B (Next of kin or executor)
Cremation form C must be completed by a doctor who had attended the deceased in advance of the death. It is important that the doctor had seen the deceased no more than 30 days before the date of death and must also have seen the deceased after death. If this is not the case then the death must be reported to the coroner. It is also important to note that all deaths which occur in a Nursing Home environment must be reported to the coroner by the doctor. Our staff will liaise with the doctor to ensure completion of this form.
Cremation Form C (Doctor)
Cremation form D is a form issued by the coroner. This form replaces form C in the event of a sudden death or if the doctor is not in a position to sign form C. Our staff will liaise with the Coroner to ensure completion of this form.
Cremation Form D (Coroner)
Please note the only form required by the family of the deceased is cremation form B. This must be completed and signed by the next of kin or the executor of the will.
